Catalyzing Rural Health Transformation

MeHAF announces a new request for proposals (RFP), issued September 14, 2017. This RFP solicits applications for rural communities to develop collaborative, multi-sector partnerships and engage members of the local community to catalyze innovative care delivery strategies that aim to improve and strengthen their local rural health system with an eye toward sustaining access to essential services in the long term. Objectives are to:

  • Support regional health care delivery and resource allocation discussions during a time of significant change and resource challenges;
  • Engage local communities to define and prioritize their needs and propose solutions;
  • Leverage public and private capacity and resources; and
  • Advance individual and population health, not just the treatment of disease.

This opportunity is open to organizations located within or serving one or more rural communities where in-patient hospital services are limited to 25 or fewer beds or there are none.

MeHAF will fund up to five (5) one-year grants of up to $30,000 each. Successful grants may be eligible to compete in late 2018 for up to three (3) one-year acceleration grants of up to $75,000.
