Beginning January 1, 2010, Integration Initiative and Cost Containment Grantees will be submitting their reporting requirements via MeHAF's Grantee Online Reporting page. To go to your account page, click here.

Inititative-based Grants

Reporting requirements may vary from grant round to grant round. Please consult your grant contract for specific requirements.

Discretionary Grants

Fill out a Discretionary Grant Report Form. Please note there is a simplified report form for discretionary grant funding for meeting support.

Complete the narrative and financial reports. Note that narrative and financial reporting forms are separate documents. Please consult section 5 of your Discretionary Grants Contract for your project's specific requirements.

  • Reports may be submitted electronically in a Microsoft compatible* format to cluce@mehaf.org, but hard copies must be sent to the Foundation at the address above.
    * People using Apple Computers may need to convert their work to Microsoft versions.

Foundation-initiated (ad hoc) Grants

Foundation-initiated (ad hoc) Grants are outside the Foundation's normal grantmaking programs and timelines. Reporting requirements are specific to each project. Please consult your grant contract or the Foundation's Grants Manager for your requirements.

Request for Modification to Grant
 
Grantees that anticipate changes in their project work plan, project end date (no-cost extension), personnel changes or budget modifications, must contact the Foundation. Requests for changes should be directed to the Program Officer assigned to the grant. Grantees are to complete the Request for Modification to Grant form. Any changes that impact the budget, must also include a revised budget. Changes must be approved in advance and in writing.
